INSURANCE PLANS WE PARTICIPATE WITH
Insertion of Bio-identical Testosterone Pellets are not covered by insurance because these pellets are a custom compounded medications and are therefore not on insurance carriers formulary of medications.
However, the Consultation Visit, Labs, and Therapeutic Phlebotomy are typically covered by most insurance plans. We participate with the following insurance carriers:

CANCELLATION POLICY & MISSED APPOINTMENTS
Due to the increased volume of patients, we have had to develop a more stringent Cancellation & Late Policy. We realize that circumstances may cause you to arrive late or miss an appointment.
Existing patients and clients should arrive 5 minutes prior to scheduled treatment time to check in.
Should you need to cancel, please contact us within 48-business hours prior to your scheduled appointment. By doing so, you make it possible for us to see another patient who might otherwise have to wait to be seen.
ALL CANCELLATIONS WITH LESS THAN 48-BUSINESS HOURS NOTICE WILL RESULT IN A $25.00 FEE.
ALL MISSED APPOINTMENTS WILL RESULT IN A $25.00 FEE
If you have 2 or more missed appointments or fail to cancel within 48-business hours:
You may be charged the full amount of your appointment / treatment

The above courtesies enable us to compensate our employees for their time, to help cover costs of opened supplies / pre-drawn medications, and maintain a higher availability of our time for you as well as others. By signing in and/or scheduling an appointment, you are agreeing to our cancellation policy.
LATE FOR AN APPOINTMENT
Please call if you know you are going to be late, so that we may try to rearrange our schedule to accommodate you or if necessary reschedule your appointment.
Should you arrive 10-minutes late, you may be asked to reschedule.
Late arrivals of less than 10-minutes may result in a shortened appointment / treatment time.
Please be aware that if you have 2 or more late arrivals that cause us to reschedule your appointment you may be:
Charged a fee of $25.00
Charged the full amount of your appointment / treatment
If you purchased a package, a treatment may be deducted off of your purchased package
